ESSENTIAL FUNCTIONS OF JOB:
* Develop reentry plans for inmates with substance use history.
* Provide procedural guidance on COMPASS enrollment.
* Check Promise status and initiate reactivation when necessary.
* Coordinate reentry services along with assigned Drug & Alcohol program counselor.
* Send referrals for Drug & Alcohol services when necessary.
* Work with Dauphin County Prison treatment staff to obtain weekly Needs Reports to identify individuals in need of substance use treatment/reentry services.
* Promote effective communication among staff, courts and agencies involved with reentry.
* Identify those requiring identification and/or driver's license services through coordination with classification treatment staff via provided Needs Report.
* Maintain weekly and monthly spreadsheets detailing state identification or driver's license status, earliest release dates and proof of identification verification.
* Document state identification and/or driver's license status in OMS.
* Ensure issuance of Photo Camera Card upon release from Dauphin County Prison for all eligible.
* Assist in obtaining identification verification to include attainment of birth certificate and/or social security card.
* Work with community partners to identify, verify and secure housing and treatment options.
* Maintain post release contact with treatment providers and clients to enhance recidivism tracking.
* Prepare monthly progress reports detailing contact with clients and treatment providers to include client contact with law enforcement.
* Establish and maintain documented communication with probation/parole during case planning.
OTHER DUTIES OF JOB:
* Attends meetings, seminars and training courses as required to boost core knowledge and treatment trends.
* Performs other job-related duties as required.
SUPERVISION RECEIVED:
* The Reentry Navigator receives occasional instruction and supervision from the Deputy Warden of Treatment and Director of Unit Management.
SUPERVISION GIVEN:
* The Reentry Navigator does not directly supervise.
WORKING CONDITIONS:
* Works indoors in limited workspace with fluctuating temperatures/ventilation and adequate lighting.
* Works indoors with frequent exposure to noise, disruptions, and stress.
* Normal indoor exposure to dust/dirt.
* Travels occasionally to various County offices and organizations in the community.
PHYSICAL/MENTAL CONDITIONS:
* Must possess ability to record, convey and present information, explain procedures and follow instructions.
* Must be able to sit for long periods throughout the workday, with intermittent periods of standing, walking, bending, twisting, reaching, stooping and grasping as necessary to carry out job duties.
* Dexterity requirements range from simple to coordinated movements of fingers/hands; feet/legs; torso necessary to carry out job duties.
* Sedentary work, with occasional lifting/carrying of objects with a maximum weight of twenty pounds.
* Must be able to cope with the physical and mental stress of the position.
* Must be able to pay close attention to details and concentrate on work.
* Must demonstrate resourcefulness in decision-making and problem-solving.
QUALIFICATIONS:
* EDUCATION/TRAINING
* Bachelor's degree from an accredited college or university, with major coursework in social sciences, psychology, education or a related field; plus
* It is expected that the successful candidate for this position will achieve and maintain any and all certifications mandated relevant by Dauphin County Prison for position.
WORK EXPERIENCE:
* Two (2) years' experience in psychological, educational, or vocational counseling or social work, preferably in a prison setting.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
* Knowledge of principles and practices of modern human services and case management.
* Knowledge of principles and practices of modern corrections.
* Knowledge of basic principles of administration and supervision.
* Knowledge of local and regional community-based organizations, religious and civic groups, public and private social-service and education providers.
* Knowledge of Prison classification system.
* Knowledge of pertinent Federal, State and County Prison Board policies.
* Proficient in the operation of personal computers and other office equipment as necessary.
* Skilled in interviewing and assessing persons of diverse age, culture, and educational attainment.
* Skilled in counseling and conflict resolution and coordination of treatment & security protocols.
* Skilled in coordinating the reciprocal agendas of treatment and security.
* Skilled in presenting and narrating presentations of public information for audiences of diverse age, culture and educational attainment.
* Ability to maintain confidentiality regarding sensitive information and records.
* Ability to understand and carry out complex oral and written instructions.
* Ability to assess and timely/accurately report (orally and in writing) extraordinary events and emergencies.
* Ability to implement routine and emergency procedures, timely and accurately; ability to exercise independent judgment, within stipulated bounds.
* Ability to work accurately and confidentially in an inherently stressful environment.
