MINIMUM JOB REQUIREMENTSHigh school graduate or GED equivalent.Available for all shifts/assignments and, when required, able to work evenings and weekends.CPR certification is required on or within 90 days of hire.PREFERRED QUALIFICATIONSTwo (2) years previous experience as Referral Coordinator.Previous medical field experience and/or referral processing, data entry.Tribal Clinic experience.Multi-lingual.SUMMARYReferral Coordinator plays a key role as a part of the clinical team. The RC is responsible for assuring continuity of care for those patients with a referral order, by coordinating timely specialty or ancillary appointments. The referral process is a multistep process and the RC is responsible to make sure that the process is followed appropriately and timely. The RC works as a liaison between the primary care provider, insurance companies, specialty providers, diagnostic services, and patients. The RC is responsible for data entry of referrals information and documental of all referral interventions.POLICYReferral Coordinator must perform duties in line with the Health Insurance Portability and Accountability Act (HIPAA). It is the responsibility of the RC to know his/her limitations and abide by the regulatory standards.SCOPE OF PRACTICEWorks under the direction and supervision of the Clinic Coordinator. Referral Coordinator (RC) processes internal and external referrals, tracks patient compliance, and procedure reports.PRINCIPAL ACTIVITIES & RESPONSIBILITIES* Process referrals according to level of urgency in timely manner.* Entry of all referral information into specialized reports. Documentation of all interventions to include date and time.* Documents necessary information pertaining to referral order in patient's electronic health record.* Submits and obtains authorizations to insurances, tracks the status of authorizations, and once approved, documents procedure approval number and authorization period in the referral tracking system.* Coordinates between patient and specialty provider to schedule appointment. Provides patient with special instructions as needed.* Communicates with primary care provider to inform of any delays or barriers to care.* Communicates with patient if any delays in scheduling appointment.* Monitors patient compliance and re-schedules appointments as needed.* Responsible to track reports and to make sure that all reports are received in a timely manner. Close referrals once the report is received, with documentation in patients' chart.* Works with primary care provider to work on re-processing authorization denials.* Manage referrals at every stage to avoid backlog.* Collaborates with all clinical staff to identify patients that may benefit from specific program enrollment due to patient conditions and/or needs.* Send trying to reach you letters to patients when unable to contact by phone.* Handle all incoming calls for referrals in a courteous manner. Provides callers with an answer to their questions as best as possible, seeking additional help by provider as needed.* Seeks assistance of supervisor when unable to process referrals in timely manner to avoid backlog and delays in care for patients.* Actively provides willingness to help and maintain professional relationships with coworkers.* Performs other duties as needed within scope of practice.* Responsible for having current CPR card.* Attends all staff meetings.* Observes and acknowledges all TRHC policies and procedures.* Always greets patients, visitors and providers in a courteous and respectful manner; responds to patients with empathy and positive interpersonal skills; consistently handles all requests in a positive manner.PHYSICAL & MENTAL DEMANDS* Able to lift/move up to 40 pounds, move from place to place, and stand for long period of time.* Ability to do math, organize and prioritize workload, work effectively and efficiently under stress.* Ability to supervise, multitask, understand and follow instructions.* Ability to proficiently read, write, speak and understand English.MINIMUM JOB REQUIREMENTS* High school graduate or GED equivalent.* Available for all shifts/assignments and, when required, able to work evenings and weekends.* CPR certification is required on or within 90 days of hire.PREFERRED QUALIFICATIONS* Two (2) years previous experience as Referral Coordinator.* Previous medical field experience and/or referral processing, data entry.* Tribal Clinic experience.* Multi-lingual.KNOWLEDGE, SKILLS, AND ABILITIES* Excellent English oral and written communication skills.* Excellent knowledge of medical clinic protocols.* Ability to complete forms and documents.* Ability to prioritize and perform multiple tasks.* Ability to maintain confidentiality.* Ability to appear for work at scheduled time.* Ability to maintain professionalism and composure.* Ability to accept constructive criticism.* Excellent knowledge of laws and regulations governing medical records and medical office management.* Ability to provide excellent internal and external customer service.SAFETY* Ensure compliance with policies and procedures related to safe work practices.* Uses all appropriate equipment and/or tools to ensure workplace safety.* Immediately reports unsafe working conditions.* Follow all infection control procedures, including blood-borne pathogen protocol.* Be familiar with and know the locations of the Safety Data Sheet Binder.PRIVACY/COMPLIANCE* Maintains the privacy and security of all patient, employee, and volunteer information and access to such information.
