Referrals Coordinator - Northwell Health : Job Details

Coordinates follow-up care for patients post discharge from the Emergency Department / Inpatient / Outpatient settings. Ensures patients obtain appropriate appointment/ speciality referral based on their care needs. Job Responsibility Responsible for accurately and responsively performing varied adminsitrative tasks and problems of moderate complexity. Solves or escalates operational problems and technical issues to senior team members and management; modifies existing tools or processes. Coordinate follow-up care for patients based on their referral needs (i.e. specialty care, primary care, preventative care, etc.). Utilizes multiple scheduling systems and tools (i.e. EMR, Registration / ADT systems, List. App, Excel, Soarian, Followmyhealth App, telephone, etc.) to schedule and manage appointments. Provide exceptional customer experience for patient, family, visitor, care team members including service recovery as needed.
