Oasis Rehabilitation and Nursing has an opportunity for a Registered Dietitian.Full-time with Benefits!Health InsuranceDental InsuranceVision InsurancePaid time off401k matchingDuties Include:AssessmentsCare planningWeight monitoringGood organizational skills and communication skills needed.Requirements:Must be registered with CDRCertified Dietician/NutritionistResponsibilities Include:Evaluate the nutritional status of patients by reviewing pertinent information obtained from the electronic medical record, the hospital discharge papers, the interdisciplinary health care team, and familyApply knowledge of nutrition screening assessment to specific patient populations to define their nutritional needsDocument pertinent findings in the medical record as per policyProvide quality nutrition counseling to patients, family members as indicatedImplement and evaluate nutrition care plans for effectiveness; recommend changes as neededUse professional judgment to adapt as well as apply guidelines, protocols plus professional standards of care to the new changing needs of the patientEvaluate the need for enteral and parenteral nutrition support; providing recommendations as indicatedParticipate in interdisciplinary care plan meetings; communicating effectively with members of the health teamComplete Minimum Data Set (MDS) documentation as requested by the facilityAssist with discharge planningMenu planning as neededKnowledge of SIGMA Care preferredPay: $38.00 - $40.00 per hourJob Type: Full-time
