Registered Dietitian - Community Care Physicians : Job Details

Community Care Weight Management & Metabolic Care is seeking an experienced Registered Dietitian!Summary: Provide nutritional counseling to patients as part of the Community Care Weight Management & Metabolic Care clinic.Essential Functions include, but are not limited to:•Plan, prioritize and organize information for patient care activities. •Perform nutrition assessment and counseling for patients.•Provide Medical Nutrition Therapy to patients.•Select or develop all necessary patient education handouts necessary for outpatient RD. •Nutritional counseling to include assessing nutritional needs, diet restrictions and current health plans to develop and implement dietary-care plans. •Advise patients and their families on nutritional principles, diet modifications and food selection and preparation. •Counsel individuals and groups on basic rules of good nutrition, healthy eating habits and nutrition monitoringThe candidate does need to be willing to facilitate group nutrition classes, during the day and occasional weekday evenings.Experience in obesity management and educationNYS License to practice as a Registered Dietitian without restrictions Ability to engage in critical thinking and clinical problem-solving.Work with all departments and sites in a collaborative manner.Possess strong clinical and interpersonal skills.Possess excellent organizational skills and keen attention to detail.Proficiency in the use of the Microsoft Office suite, especially Excel, Outlook and Word.Familiarity with electronic medical records and ambulatory care environment.
