Our client seeks a passionate and skilled Registered Dietitian to provide exceptional nutrition care and education for patients. This role involves evaluating nutritional status, creating individualized care plans, and collaborating with an interdisciplinary team to ensure the well-being of patients. What you will be doing:
- Assessment: Gather patient data to evaluate nutritional needs, including intake, texture, and overall tolerance. Identify potential problems requiring follow-up or referral.
- Planning Care: Develop care plans that align with American Dietetic Association (ADA) standards and integrate interdisciplinary care. Collaborate with patients and families to establish goals.
- Implementation:
- Ensure meals meet nutritional industry standards and dietary requirements.
- Educate patients, families, and staff on dietary regimens and nutritional needs.
- Work closely with physicians to align nutrition plans with medical care.
- Document interventions accurately and timely in patient records.
- Oversee meal service delivery, including coordination with vendors.
- Evaluation: Monitor nutritional intake, patient weight, and progress. Adjust care plans as necessary based on assessments and patient feedback.
- Leadership: Actively participate in quality improvement initiatives, address food service issues, and uphold high standards in care delivery.
- Interdisciplinary Collaboration: Engage as a vital member of the care team, participating in staff meetings and patient care planning.
What you must have:
- Bachelor's degree in Nutrition from an ADA-accredited institution.
- Registered Dietitian credential through the Commission on Dietetic Registration.
- Minimum of 1 year of experience as a Dietitian.
- Experience in clinical dietetics with a focus on geriatric care.
- Reliable transportation, a valid driver's license, and the minimum state-required liability auto insurance.
- Must pass a comprehensive criminal background check, including exclusion lists and drug screening.
- Up-to-date immunizations and medical clearance for communicable diseases.
- Certified Diabetes Educator (CDE) status preferred or CDE-eligible within 24 months.
- 1 year of experience working with a frail or elderly population (training provided if not already experienced).
- Strong interpersonal and teaching skills, particularly with geriatric patients and their families.
- Basic computer proficiency, including email and electronic medical records.
