By assessing and evaluating nutritional statuses and developing personalized care plans, you will increase the participants' quality of life. Your work will involve collaboration with interdisciplinary teams, ensuring a seamless integration of dietary care with other health services. - Conduct comprehensive nutritional assessments by collecting patient data and evaluating their nutritional needs.
- Develop effective care plans that align with the American Dietetic Association standards and incorporate interdisciplinary strategies.
- Implement and manage nutritional care, including coordinating meal services and ensuring conformity to industry standards.
- Educate participants, their families, and caregivers about nutrition and dietary plans.
- Monitor and evaluate participants' nutritional intake and make necessary adjustments to care plans.
- Actively participate in interdisciplinary team meetings and contribute to quality improvement processes.
- Collaborate with primary care physicians and support staff to integrate care solutions.
- Document nutritional interventions accurately and ensure records meet departmental standards.
Requirements:
- Bachelor's degree in Nutrition from an ADA-accredited institution.
- Minimum of 1 year of experience as a Dietitian, with clinical dietetics exposure, especially for the geriatric population.
- Registered member of the ADA and registered by the Commission on Dietetic Registration.
- CDE status preferred, but not required; must be eligible for CDE within 24 months.
- Experience with a frail or elderly population is preferred. Training provided if needed.
- Reliable transportation, valid driver's license, and appropriate auto insurance.
- Medically cleared for communicable diseases with up-to-date immunizations.
- Successful completion of a comprehensive background check.
