*Registered Dietician / Dietitian - PACE Clinic**Location:* Amityville, NY*Pay: *$47.00-$50.00 DOE*Schedule:* Monday - Friday 9am - 5pm (1 hour lunch)*Assignment Type: *Long term temp*Requirements*: Active RDN and Bachelor's Degree*Job Duties: *The Dietitian will be responsible for providing comprehensive assessment and nutrition treatment and coordination of 24-hour care for participants as part of the interdisciplinary team.* Performs initial face-to-face and telehealth assessments and evaluates the nutritional status of individual participants utilizing nutritional guidelines, professional knowledge, and interviewing skills to provide quality care.* Evaluates participants' physical condition and calculate therapeutic diets to meet the specific requirements of individual patients. Recommends to the team the plan of care and status of participant progress.* Provides clinical recommendations and creates orders to reflect the individualized needs of each participant, under the approval of the Primary Care Physician/Nurse Practitioner.* Maintains an up-to-date file of all participants at assigned locations to include their diet order, modifications, allergies, restrictions, and likes/dislikes, as well as their Annual Assessment date* Establishes nutritional plans on assessment of participant's condition, monitors participant's progress, and recommends/revises changes in the nutritional plan for effective participant care.* Monitors participant progress and outcomes through assessment of care provided, including the need for revision to the treatment plan.
