Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
Candidate will provide oversight individuals with developmental disabilities who have a variety of medical and psychological diagnoses. Candidate will work as a Registered Nurse I in the Family Care program in the Albany, Rensselaer, Schoharie and Schenectady counties. Candidate will case manage individuals who reside in Family Care Homes throughout this region and neighboring areas.
Job Duties to include:
* Supervises the health care of individuals living in Family Care Homes by working with Family Care Providers to implement optimal care, incorporating regulations, policies and procedures governing Family Care.
* Assesses individuals on an ongoing basis, identifying needs.
* Completes, at minimum, one nursing visit every 90 days (must be flexible based on individual's needs).
* Works with Family Care Provider to coordinate appointments for primary visits, consultant services and required tests/diagnostics.
* Provides oversight to individuals when hospitalized through weekly updates/visits, including the discharge transition.
* Ensures that medications and treatments are administered according to approved policies and procedures. Provides ongoing oversight to the Family Care Provider in administering medications, retraining in medication administration practices and medication pours as applicable.
* Provides training and oversight to the Family Care Providers and Substitute Providers in the areas of medication administration, OSHA (Blood Borne and TB), Infection Control, emergency care and individual specific training needs. Also provides mandatory and as needed training to Providers as applicable.
* Coordinates community health care services, working with outside providers and locating distributors for equipment and supplies (examples: wound care, oxygen therapy, physical therapy). Also, obtains consent for procedures from consent givers or SDMC.
* Provides nursing component to the Interdisciplinary Team. Collaborates with clinicians, team members, families, and community providers. Works with Interdisciplinary Team on ways to best support Providers. Participates in home recertification process. Participates in screening process for new individuals.
* Completes incident investigations and other duties as assigned.
* Provide nursing coverage to other Family Care homes and IRAs on an as needed basis.
The Candidate must be able to perform CPR compressions on a supine individual on the floor. The Nurse demonstration of CPR compressions on the floor would ensure nurse ability to perform in worst case scenario, since we use no backboards in OPWDD.

Minimum Qualifications Must have and maintain a valid license and a current registration as a registered professional nurse in New York State.

* This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.
* At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
* Please note that starting salary for this title reflects the increased hiring salary and geographic pay differential. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 18 ($65,001), Increased Hiring Salary ($8,928), and Geographic Differential ($20,000). Geographic Differential added to the job rate ($82,656) for PS&T Grade 18.
