Job Title: Registered Nurse (RN) – Heart Failure & Heart Transplant Subdivision

Department: Cardiology

Location: Major NYC Health System

Schedule: Monday to Friday, 8:30 AM – 5:00 PM

Status: Full-Time, Outpatient Role

Overview:

We are seeking a dedicated and experienced Registered Nurse (RN) to join the Heart Failure and Heart Transplant Subdivision within the Cardiology Department of our renowned healthcare system. This outpatient role involves primarily triaging patients, managing administrative responsibilities, and providing high-quality patient care in collaboration with our cardiology team. The ideal candidate will demonstrate strong critical thinking skills, a deep understanding of heart failure and heart transplant care, and the ability to handle medication reconciliation effectively. This is an excellent opportunity to work in a fast-paced, dynamic environment, supporting patients as they navigate their heart health journey.

Key Responsibilities:

  • Patient Triage: Efficiently assess and triage patient inquiries, concerns, and symptoms, directing them to the appropriate clinical team members as necessary.
  • Medication Reconciliation: Perform comprehensive medication reconciliation for heart failure and heart transplant patients, ensuring accurate documentation of all medications, dosages, and instructions. Collaborate with the clinical team to resolve any discrepancies.
  • Clinical Support: Provide clinical support to cardiologists and other healthcare providers in managing patient care, including monitoring patient status, updating patient records, and following up on treatment plans.
  • Care Coordination: Assist in coordinating outpatient care for heart failure and heart transplant patients, scheduling appointments, and facilitating communication between patients and specialists.
  • Administrative Duties: Manage and maintain patient records, including documenting triage encounters, care plans, and medication lists. Assist with preparation of reports and follow-up communications as required.
  • Patient Education: Educate patients and their families on heart failure and heart transplant management, medication adherence, and lifestyle modifications to optimize heart health outcomes.
  • Critical Thinking: Apply clinical judgment and critical thinking skills to solve complex problems related to patient care and provide input into treatment decisions as needed.
  • Collaborative Teamwork: Work closely with the multidisciplinary healthcare team, including cardiologists, transplant coordinators, nurse practitioners, and support staff, to ensure the best possible outcomes for patients.
  • Quality Improvement: Participate in ongoing quality improvement initiatives and contribute to efforts aimed at enhancing patient care within the heart failure and transplant program.

Qualifications:

  • Education: Registered Nurse (RN) with an active, unrestricted license in the state of New York.
  • Experience: Minimum of 3 years of clinical experience in cardiology, heart failure, or heart transplant nursing preferred. Previous outpatient experience is a plus.
  • Skills:Strong critical thinking, assessment, and clinical decision-making skills.
  • Expertise in medication reconciliation and understanding of complex medication regimens.
  • Ability to manage multiple tasks and prioritize effectively in a fast-paced environment.
  • Proficiency in electronic medical records (EMR) systems.
  • Excellent communication and interpersonal skills with patients, families, and the healthcare team.
  • Certifications: Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification preferred.
  • Personal Attributes: Compassionate, detail-oriented, and able to work independently while contributing to a collaborative team environment.
