Job DescriptionMinimum Qualifications
  • Must maintain a valid New York State license and registration
Applicants must possess a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.Substitution:A bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.Duties Description
  • Posted Hiring Salary includes the sum of the statutory hiring rate ($72,032) for PS&T Grade 20, Geographic Pay Differential ($20,000) and Increased Hiring Salary ($9,688). Maximum salary includes the sum of Job Rate ($91,381) and Geographic Pay Differential ($20,000).
  • A total of 5 hours deviation from the default schedule (Hours before or after the regular workday). 4 hours a month are expected to be on a weekend day and would count toward the above deviation expectation.
PRIMARY DUTIES AND RESPONSIBILITIES:Candidate would function as Registered Nurse 2 with duties as described below. Additional duties to be assigned as necessary by the Director of Nursing/Nursing Supervisor. The candidate must be highly organized, professional, and able to self-direct. Candidate must have good interpersonal skills, consistent communication with supervisor and be able to collaborate well with other RN's and disciplines.Duties will include:
  • Assist residential nurses as hospital liaison for individuals who are hospitalized by assisting with weekly hospital visits, obtaining, and updating daily report on hospitalized individuals, collaborating with hospital team/OPWDD treatment team/family for safe discharge, establishing discharge planning meetings, advocating for individual's medical needs and coordinating care/supplies/equipment at IRA for any additional supports needed upon return.
  • Coordinate with hospitals, family, residential RN's, clinicians and providers for long-term support, palliative care and discussions surrounding end-of-life concerns.
  • Establish relationships/contacts of care manager teams at local hospitals in region. Maintain list of these contacts.
  • Follow up, obtain/retain, upload copies of hospitalization medical records into Electronic Medical Record as directed.
  • Provide nursing component to IST (Intensive Support Team), CATH (Comprehensive Adult Transition Home) committee and other interdisciplinary teams aiding strategies to support the individuals.
  • Provide training for RN's to aid in the transition of coverage of newly opened IRAs.
  • Assist with education of nurses and direct support staff as assigned.
  • Support management of emerging and existing medical/nursing issues.
  • Assisting in ensuring compliance with rules/regulations governing nursing practice in OPWDD.
  • Assist and ensuring implementation of facility policies and procedures related to nursing.
  • Participate in various meetings and serving on committees as needed.
  • Attends/completes on-line mandated DDSO training, as well as discipline specific training.
  • Completes special projects as assigned.
  • Oversees individuals' overall health care needs and works with residential nurses to implement optimal care.
  • Incident Investigation and other duties as assigned by supervisor.
The Candidate must be able to perform CPR compressions on a supine individual on the floor.   • This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.
  • At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
  • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
