Job Description: Registered Nurse (RN) – Heart Failure & Heart Transplant Team

Location: New York, New York

Department: Cardiology – Outpatient Setting

Position Overview:

We are seeking a skilled and dedicated Registered Nurse (RN) to join our dynamic Heart Failure and Heart Transplant Team in the Cardiology Department of our large health system in New York. The ideal candidate will have a strong commitment to patient care, a diverse clinical background, and the ability to quickly adapt in a fast-paced outpatient environment. This position will primarily focus on triaging patient inquiries, providing essential clinical assessments, and facilitating communication between patients and healthcare providers. The RN will also be responsible for administrative tasks, including documentation, medication reconciliation, and the management of forms.

Key Responsibilities:

  • Patient Triage:
  • Field incoming patient calls and assess clinical concerns, including symptoms such as shortness of breath, leg swelling, and other complaints related to heart failure or transplant care.
  • Use clinical judgment to determine the urgency of the situation and direct patients to the appropriate level of care (e.g., emergency department, provider office).
  • Collaborate with physicians to share important information regarding patient conditions and symptoms.
  • Recognize key clinical indicators, such as abnormal INR levels and lab results, and communicate findings to the medical team.
  • Clinical Skills & Patient Management:
  • Conduct detailed assessments and gather pertinent information about patients' medical histories, including medications and existing conditions.
  • Manage medication reconciliation and question discrepancies to ensure accuracy.
  • Assist with administrative tasks related to patient care, including but not limited to faxing documents, confirming appointments, and responding to messages.
  • Assist with completing disability forms, clearance letters, and other necessary documentation for patients.
  • Documentation & Administrative Duties:
  • Ensure accurate and timely documentation in the electronic health record (EHR) system.
  • Coordinate communication between patients and providers, ensuring the smooth flow of information.
  • Monitor and track patient care plans and appointments to ensure appropriate follow-up and care continuity.
  • Collaborative Work Environment:
  • Work closely with the multidisciplinary heart failure and heart transplant team, including cardiologists, nurse practitioners, and other healthcare professionals.
  • Maintain open communication with physicians to address patient concerns and escalate issues as needed.

Qualifications:

  • Education & Licensure:Valid and active Registered Nurse (RN) license in New York State.
  • Associate's or Bachelor's degree in Nursing (ADN or BSN) from an accredited program.
  • Experience:Minimum of 2 years of inpatient nursing experience, preferably in cardiology, emergency room, or a critical care setting.
  • Experience in triaging patient concerns and providing clinical advice.
  • Skills & Abilities:Strong clinical knowledge and ability to assess and triage medical concerns effectively.
  • Excellent communication skills and the ability to engage with patients and healthcare providers professionally.
  • High attention to detail and accuracy in documentation and medication reconciliation.
  • Ability to work efficiently in a fast-paced, high-volume outpatient setting.
  • Comfort with administrative tasks, including form completion, faxing, and patient messaging.
  • Preferred:Previous experience working in a cardiology or heart failure/transplant team is a plus.
  • Familiarity with Epic EMR.
