About your Role:As an ACT Nurse, you'll be a vital support for individuals in achieving their treatment goals, helping them thrive in their communities. You'll serve clients directly in their homes, workplaces, and neighborhoods, providing compassionate medical care and support services such as taking vitals, conducting lab tests, and offering medication management through close coordination with their medical providers. You'll be part of a dedicated, multidisciplinary team focused on empowering clients to live happy, healthy, and productive lives while staying connected to friends, family, and their communities. You'll also use a company vehicle to transport clients as needed, and take part in a rotating on-call schedule every 7-10 weeks, for which you'll earn additional PTO. What is the ACT Program? The Assertive Community Treatment (ACT) model, is one of the most widely recognized evidence-based approaches for supporting adults with severe mental illness. By providing person-centered assistance-including housing, recovery, vocational support, nursing, psychiatric, and therapeutic services-ACT helps clients transition to lower levels of care or move toward greater independence, enhancing their quality of life.Does the following apply to you?
  • Licensed as a Registered Nurse (RN) OR Licensed Practical Nurse (LPN) in Louisiana
  • Experience providing treatment to children, youth, or adults with severe and persistent mental illness and/or substance abuse illness
  • Holds a valid state driver's license in good standing
  • Psychiatric experience is a plus
