Work Hours:

  • Total Clinical Hours: 32-40 weekly
  • Hours of Operation: Monday-Thursday: 9 a.m. - 12 a.m Sunday: 5:30 p.m. - 10:00 p.m.

  • Shifts: Shifts can vary and will be arranged based on discussions with the scheduling manager (5 hours, 8 hours, or potentially 12 hours)

Position Overview:

  • Illuminate Patient-Centered Care: Utilize patient-centered care to help build rapport and improve the health of acute care patients. Complete acute care patient lists and follow up with patients to ensure they are compliant with medications and follow-up visits.
  • Champion Continuity of Care: Complete hospitalization follow-ups using the Azara system. Manage diabetic care by obtaining lab results, ensuring follow-ups with ophthalmology and vision, and ensuring diabetic foot exams are completed, including follow-ups with podiatry.
  • Navigate Complex Communications: Handle complicated messages from providers regarding patients' health. Conduct nurse visits, including injections, PPD tests, medication teaching, and insulin teaching.
  • Collaborate with Specialty Providers: Work with specialty providers as the center grows. Complete intakes and scribing, obtain medical records, order diagnostic imaging, and heart monitors, offer education, follow up on heart monitor results, and order cardiac procedures at outside facilities.
  • Streamline Patient Support: Order supplies for patients via Parachute Health. Write letters for providers, complete phone triage for patients with emergent concerns

Minimum Qualifications:

  • 3+ years of ER experience
  • RN associate's degree
  • Team player and strong communicator

Preferred Qualifications:

  • 7+ years of ER experience
  • 1+ years ECW or another EMR experience

Pay and Benefits:

  • $37 per hour, commensurate with experience
