Greenlife Healthcare Staffing is seeking a dedicated Registered Nurse to fill an opening within an Assisted Living Facility located in NYC & Hudson Valley. Requirements of a Registered Nurse:
- Must have at least 6 months of clinical experience.
- Must have an active New York state license
- Must have an active driver's license
Schedule of a Registered Nurse:
- This is a Full-time position
Compensation and Benefits of Registered Nurse:
- Competitive salary ranging from $120,000 to $125,000 per year.
- 4 weeks of vacation.
- 5 personal days.
- 8 sick days per year.
Essential Job Duties of a Registered Nurse:
- Performs a home care assessment and re-assessment of patient and environment to determine home health care needs. As indicated, uses the Oasis assessment tool.
- Calls the home care coordinator or supervisor with verbal report of initial findings.
- In conjunction with the family and physician and/or authorized practitioner and Supervisor develops and implements the Plan of Care based on the assessment.
- Provides skilled nursing care as outlined in the Plan of Treatment to include the following: Nursing services, treatments and preventive procedures including those requiring substantial specialized skill and ordered by the physician or authorized practitioner.
- Initiates preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety.
- Evaluates the effectiveness of his/her nursing service to the individual and family.
- Observes signs and symptoms and changes in the patient's physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary.
- Reports all patient changes and information to the supervisor or home care coordinator on a timely basis.
- Submits a record of visits/hours worked each day.
- Participates in case management conferences with other team members.
- Teaches, supervises, and counsels the patient and caregivers regarding the nursing care needs and other related problems of the patient at home.
- Documents services rendered, changes noted in the patient's condition, and/or family and home situation, and evaluate patient response to treatment.
- Documentation of all care provided is in accordance with company policies and procedures.
- Attends in-service and continuing education.
- Participates in Performance Improvement activities.
- Orients and supervises home health aide staff and documents supervision according to company policies and procedures.
- Observes client rights and confidentiality.
- Observes infection control and standard precautions.
- Observes Advance Directives, as indicated.
- As indicated, reports client changes to supervisor.
- If assigned to an adult care facility, completes statistical data.
- If full-time or assigned as full-time to adult care facilities, coordinates total patient care in cooperation with the home care coordinator and/or supervisor.
- If assigned to an adult care facility, the Community Health Nurse appropriately responds to emergencies for all residents of the adult care facility, in a timely, professional, caring manner.
- All other duties as assigned.
