Job Summary:
The Call Center Registered Nurse (RN) serves as the primary point of contact for patients calling with clinical matters, utilizing the nursing process and physician-approved protocols to deliver care via telephone. The RN provides accurate and respectful patient interactions while maintaining confidentiality and adhering to organizational policies and applicable laws. The role involves detailed attention to clinical tasks, patient education, and effective communication with patients and interdisciplinary care teams. This position is hourly exempt.
Main Responsibilities:
Patient Care
* Provide patient care demonstrating nursing knowledge and technical competence, addressing changes in patients' conditions with appropriate actions.
* Promote patient independence by teaching patients and families about conditions, medications, and self-care skills, and addressing their questions.
* Formulate and coordinate care plans with patients, caregivers, and interdisciplinary teams to meet individual healthcare needs.
Telephone Triage and Coordination
* Perform telephone triage with diverse patient populations, referring for medical management as necessary.
* Follow up with patients to communicate test results, provide education, and escalate medical management when appropriate.
* Respond to home care requests from providers, making referrals and addressing additional patient needs as identified.
Compliance and Documentation
* Ensure adherence to infection control policies, medication administration protocols, and controlled substance regulations.
* Document all referral activities and interactions in a complete, accurate, and timely manner in accordance with established standards.
* Maintain compliance with HIPAA and other regulatory standards related to patient care and confidentiality.
General Responsibilities
* Provide health teaching and counseling to patients and families, serving as an advocate for their needs.
* Use effective communication and judgment to assess patient needs and facilitate information exchange with providers and other team members.
* Participate in other tasks as assigned by the supervisor.
Education and Qualifications:
* A current, unrestricted NYS Registered Nurse License required
* CPR certification required (can be provided)
* 1-3 Year of experience working in license
* The ability to work flexible hours -- some evenings are required
* The ability to communicate fluently in both Spanish and English is preferred
Physical Requirements:
* Must be able to hear and communicate with clients and staff via phone, web media, and in person.
* Must be able to lift up to 40 lbs
* Must be able to sit for extended periods of time
* Must be able to kneel, crawl, or bend into awkward spaces
* OSHA Level 1
