We are searching for a Registered Nurse Care Manager (RNCM) to serve as the quarterback for our patients, ensuring their unique needs are assessed and met with excellence and empathy.

As an RNCM, you'll work under the guidance of the Director of Operations, serving as the main advocate and resource for patients, their families, and providers. You'll coordinate care, develop individualized plans, and manage resources to deliver holistic, client-centered support. We place a high value on clear and concise communication, fostering trust and respect within our team and with clients. Your strong writing and organizational skills will support seamless care coordination and meticulous documentation, ensuring the best outcomes for our patients.

What We Offer:

As an RNCM at Alliance Homecare, you'll thrive in an organized and collaborative setting that prioritizes open communication. This is a full-time role, Monday to Friday, 9 AM–5 PM, with occasional on-call duties. You'll spend approximately 70% of your time in the field at clients' residences, supported by a dedicated team and robust resources.

What You'll Bring:

• A Bachelor of Science in Nursing (Master's preferred)
• An active, unencumbered nursing license in the state of New York
• Minimum of 5 years of nursing experience
• Strong organizational, problem-solving, and interpersonal skills, with a commitment to clear and compassionate communication
