FlexStaff is seeking an experienced RN to work for one of our external clients, a community services non-profit organization serving people with developmental disabilities for their residential program, located in Jamaica, Queens, NY.

This is a permanent, full-time job, Monday - Friday, 4pm-12 am, OR 12 pm -8pm

Additionally the nurse provides on call triage coverage via cell phone. The on-call process requires that the RN/Nurse Case Manager is available to respond to all calls and triage the call within thirty minutes, from 6AM to 6PM Monday through Friday, and Weekend on-call rotation on Saturday Morning from 6am to Monday morning 6am. For the weekend on call, the nurse will receive a comp day to use after they complete the weekend on-call triage.

There is a volunteer option for weeknights, Monday to Friday from 6pm to 6am, where the nurse is compensated for each night they are on call.

In this role you will be responsible for developing plan of care and coordinating the delivery of health care services for persons receiving services in an assigned program unit.

An ideal candidate would be a graduate of an accredited program of professional nursing and have a valid license and current registration as a Registered Nurse in the State of New York.

Responsibilities include, but not limited to:

* Coordinates routine and specialized healthcare services to meet the assessed needs of individuals in the program.

* Assesses and updates healthcare outcomes at least semi-annually, maintaining health profiles.

* Develops and oversees nursing care plans, educating individuals, caregivers, and families on implementation.

* Trains and supervises staff on treatment plans as delegated by the RN.

* Assigns staff to healthcare tasks based on procedure complexity and staff experience, ensuring proper training and monitoring.

* Evaluates healthcare effectiveness, making recommendations for alternative care when needed.

* Serves as a liaison, communicating health information among team members, families, and service providers.

* Maintains documentation in the electronic health record, charting as needed for acute or ongoing medical issues.

* Provides health-related training to individuals, staff, and team members.

* Ensures AMAP staff certification and recertification, coordinating with the Nursing Educator and Residential Manager.

* Provides on-call triage coverage and onsite visits when necessary.
