Job DescriptionFlexStaff is seeking an experienced RN to work for one of our external clients, a community services non-profit organization serving people with developmental disabilities for their residential and habilitation programs, located in Manhattan, NY.This is a permanent, full-time job, Monday - Friday, 9a-5p. or 8a-4p.Additionally the nurse provides on call triage coverage via cell phone. The on-call process requires that the RN/Nurse Case Manager is available to respond to all calls and triage the call within thirty minutes, from 6AM to 6PM Monday through Friday, and Weekend on-call rotation on Saturday Morning from 6am to Monday morning 6am. For the weekend on call, the nurse will receive a comp day to use after they complete the weekend on-call triage.There is a volunteer option for weeknights, Monday to Friday from 6pm to 6am, where the nurse is compensated for each night they are on call.In this role you will be responsible for developing plan of care and coordinating the delivery of health care services for persons receiving services in an assigned program unit. An ideal candidate would be a graduate of an accredited program of professional nursing and have a valid license and current registration as a Registered Nurse in the State of New York.Responsibilities include, but not limited to:
- Develops plan of nursing services and oversees the implementation of the health care plan by educating person's receiving services, DSP/Caregivers and families in the plan of nursing services.
- Educate, monitor and supervise staff in carrying out ordered treatment plan related to health needs as delegated by the RN.
- Assigns Direct Care staff to perform health care treatments/procedures after having taken into consideration the complexity of the procedure(s), the skill/experience of the staff and the health condition(s)/status of the person receiving services. Initially trains the staff in the task/activity and regularly observes/monitors to ensure all treatments/procedures are performed correctly.
- Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.
- Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers.
- Completes and maintain all required written documentation on the approved electronic health record system. Charts health care progress as indicated by acute illnesses and/or complex ongoing medical problems. required
- Conducts general and individual specific health related trainings for persons receiving services, direct care staff, and other team members as needed.
