Gulfside Healthcare Services is expanding our Hospice Service Line for services in Hillsborough County. We are posting this unique opportunity to come grow with Gulfside in our new Hillsborough County operations. We estimate the start date of this position to be mid-end of February with additional positions to be added as we grow. Please contact our Human Resources Team for additional information on our expansion.Reporting to the Clinical Care Manager, the Registered Nurse is responsible for providing quality care to patients with Gulfside Healthcare Services, in the field. The field includes patients who reside in private residences, assisted living facilities, and skilled nursing homes. The RN Hospice Field utilizes assessment, intervention, and education to maximize patient comfort and to enhance the quality of life for the patient/family. The RN Hospice Field oversees and case manages the needs of the patients assigned to them. This includes providing supervision for the LPN's and CNA's, and other assigned staff in the field. The Registered Nurse is responsible for maintaining his/her own clinical and mandatory education requirements and for meeting the established standards of their profession as well as those accountable for his/her practice, assigned duties/patient care, and is an advocate for patients/families and their profession.$5,000 Sign-On BonusShift: Monday - Friday 4pm - 1am EDUCATION AND QUALIFICATIONS:Currently licensed in the State of Florida as a registered nurseMinimum of one (1) years nursing experience in a hospital, home health or hospice setting preferredStrong ethical standards and an appropriate professional demeanorShould possess sound critical thinking, time management, technical skills, analytical ability, and good judgmentThe ideal candidate is a well-organized and self-directed individual who is a team playerMust have reliable transportation to perform tasks and responsibilities in a timely and appropriate fashion. Must be able to provide proof of automobile insurance and have a valid Florida driver's license ESSENTIAL JOB RESPONSIBILITIES:Case manages the needs of the patients and families in the field assigned to themPerforms ongoing assessment of physical, psychological, social, and spiritual needs of the patient/family unit and initiates appropriate interventionProvides and/or oversees supervision of P.N.'s, C.N.A.'s, and other assigned staffTakes independent action to identify problemsSupports policies and proceduresAssists in the orientation of new staff membersFunctions as a resource person; provides mentoring and guidance for co-workers and additional staffMaintains positive working relationships with team members of all disciplinesAttends to patient/family complaints and concernsCreates and promotes initiatives to develop and strengthen relationships with facility partnersTakes a proactive approach to identifying and resolving potential issuesEducates the community regarding hospice care, philosophy, and concept through individual consultation with persons in the communityCommunicates regularly with the Clinical Care Manager and continuously assesses areas for risk managementMaintains current with organizational and departmental communication via e-mail. All clinical staff are required to check their e-mail accounts at the start of each shift.Effectively communicates, collaborates, and coordinates with colleagues and management on issues related to nursing process and serviceCreates and implements nursing care plans which are reviewed daily and updated as neededConsiders financial implications in rendering cost-effective nursing careEnsures professional standards of nursing practice are maintainedCompletes required documentation prior to departing from scheduled shiftActively participates in the IDG (Interdisciplinary Group) and communicates patient needs appropriately to other disciplines when requestedCollaborates with team physician daily on the status of the patientPerforms other duties as requested by the Nurse Care Manager SKILLS AND COMPETENCIES:Maintains up to date skill sets as required to care for patientsPossesses ability to multi-task; exhibits adaptability and flexibility in meeting Gulfside Healthcare Services objectivesEnsures compliance with all patient care and documentation standards and regulationsAssists in the implementation and maintenance of the policies and procedures of the clinical department; adheres to and assists in the evaluation of the sameAssists in maintaining federal and state regulatory complianceProficient in Electronic Medical Records#GSRN1Licenses & CertificationsPreferredRN
