Job Details: A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager.The RN uses patient assessment to guide nursing care as defined by agency policy Responsibilities:
- Initiates the plan of care, makes revisions, and updates as necessary.
- Follows evidence-based clinical practice guidelines incorporating case management principles for care coordination oversight.
- Develops and implements a plan of care based on evidence-based guidelines, incorporating patient-centered goals.
- Communicates/Coordinates services with MD, patient, caregiver, interdisciplinary care team and makes appropriate referrals to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills timely when appropriate. Evaluates progress toward patient-centered goals weekly and revises plan when needed.
- Processes orders and notifies physician of patient needs and changes in condition.
- Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with agency and regulatory policy guidance.
- Assignment of HHA , Completion of HHA Duty sheet, and supervision per regulatory requirement.
- Submits complete and accurate daily activity sheets.
- Coordinates with Insurance rep for insurance authorization as needed.
- Delivers Notices of Non coverage for Medicare Patients (HHABN, NOMNNC,DENC) according to agency policy.
- Takes on-call duty nights, weekends and holidays, as assigned.
- Participates in onboarding other nursing personnel, peer reviews, and Quality Assessment and Performance Improvement activities as assigned.
- Identifies and reports all potentially unsafe situations, which could cause harm to patients/staff to manager to establish actions to minimize risks/harm.
- Accepts assignments willingly. Travels to all geographical areas.
- Documents patient progression toward clinical goals for each patient, plans care conferences on his/her patients, and conducts discharge planning in a timely manner as per Agency policy.
- Performsmedication reconciliation and management practices in accordance with agency policy and documents in the medical record.
- Educates and empowers the patient and family/significant others for safe home management and discharge.
- Takes on call duty nights, weekends, and holiday as assigned.
- Demonstrates regular, reliable and predictable attendance.
- Maintains WMC mission, vision and values.
- Performs other duties as required.
Experience:
- One year of acute care experience or related experience.
Education:
- Graduate of an accredited school of nursing.
Licenses/Certifications:
- Current licensure in New York State as a Registered Professional Nurse.
- Certified in Basic Life Support.
- Other certifications as required in specialty area.
- NY State Driver's License.
