Home Health RN Incentives: (Full-time or Part-time)

• $10,000 sign on with 2 year work commitment ($5,000 in first paycheck, remaining $5,000 after completion of 12 months of service)

Pay Range: Days: Starting Pay: $36.54 Gas mileage reimbursed at federal rate The base pay range is a good faith representation of current potential base pay for successful applicants. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity and contracts.

• Previous RN experience is preferred.
• Previous Home Health Care experience preferred.
• Minimum 1 year of medical-surgical experience.
• Primary Responsibilities: Planning, coordinating, providing and documenting the care of patients in their place of residence.
• Clinical practitioner who coordinates and implements patient care specific to Jefferson County.
• Ensures that compassionate, engaged care meets or exceeds quality standards.
• Care is provided in an efficient and safe manner consistent with the NYS licensure and Samaritan Home Health standard of care.
• Utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care, cultural, ethnic and spiritual and educational needs of the patient/family/significant other to develop, implement and revise the plan of care. In doing so, performs accurate assessments, interprets and correlates finding, establishes plan of care and implements short and long-term goals.
• The nurse observes patient progress, records pertinent observations, implements physician and nursing treatment plans and evaluates outcomes.
• The RN recognizes their responsibilities as specified by law and abides within those responsibilities.
• Provides direct supervision to LPNs and HHAs.
• Provides on-call services as required.
• BLS required.
• ACLS preferred.
• Valid NYS Driver's license and proof of auto insurance.
• Must be available weekends and Holidays and provide on-call services as required.
