Job DescriptionThe Home Health Registered Nurse is responsible for traveling to a patient's home to provide skilled nursing service in accordance with the orders of the patient's health care professional, under a plan of treatment established by the healthcare team. Principal Responsibilities
- Be responsible for the observation, assessment, nursing diagnosis, council, care and health teaching for patients, and health maintenance and illness prevention.
- Provide progress notes to the patient's provider when the patient's conditions change or there are deviations from the plan of care, or at least every 60 days.
- Direct the activities of the licensed practical nurse and supervise the aide in the home.
- Administer medications and treatment as prescribed by the patient's provider.
- Act as the coordinator of the healthcare team to maintain the proper linkages within a continuum of care.
- Perform acts in accordance with the IL Nurse Practice Act.
- Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
- Attends all training and educational requirements for role.
Education/Experience
- 1+ year experience in a clinical care setting as a nurse
- Associates Degree or Bachelor's Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
Licenses/Certificates
- Current, unrestricted state RN licensee
- CPR certification with American Heart Association
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
Knowledge, Skills and Abilities
- Able and willing to travel within branch/office coverage area.

- This job operates in a professional office environment. Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
