*Overview*We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. This role is essential in providing high-quality patient care in the comfort of their homes. As a Registered Nurse in Home Health, you will be responsible for assessing patient needs, developing care plans, and coordinating with multidisciplinary teams to ensure optimal health outcomes.*Responsibilities** Conduct comprehensive assessments of patients' health status and needs in their home environment.* Develop individualized care plans based on diagnostic evaluations and patient care requirements.* Provide direct nursing care, including medication administration, wound care, and monitoring of vital signs.* Educate patients and their families on disease management, medication adherence, and health promotion strategies.* Collaborate with healthcare providers to coordinate managed care and ensure continuity of services.* Maintain accurate documentation in compliance with HIPAA regulations and organizational policies.* Participate in quality improvement initiatives to enhance patient safety and care standards.*Experience** Current Registered Nurse (RN) license in the state of practice.* Previous experience in clinic or hospital settings preferred; exposure to managed care is a plus.* Familiarity with Level II Trauma Center protocols and emergency response procedures.* Knowledge of infectious disease care practices and principles.* Experience with diagnostic evaluation processes is desirable.* Strong skills in patient care management within various healthcare environments including OR and ER settings.* Excellent interpersonal skills to effectively communicate with patients, families, and healthcare teams.Join us in making a difference in the lives of our patients by providing compassionate home health nursing care.
