We are seeking an experienced Registered Nurse (RN) for a Home Health position to serve Eastern Lewis County, WA, and surrounding areas. The RN will provide skilled nursing care in patients' homes, promoting health, independence, and comfort. This role requires someone with at least 1 year of skilled home health experience, including proficiency with Oasis assessments. The RN will work 8-hour day shifts, visiting up to 6 patients per day with a 1:1 patient ratio, and no on-call or weekend shifts are required. Travel between patient homes within the region will be necessary, so a reliable vehicle is essential.

• Patient Care & Assessments:

  • Provide direct home health nursing care to patients in their homes, performing initial and ongoing Oasis assessments.
  • Perform physical assessments, administer medications, provide wound care, and manage patient care plans.
  • Develop individualized care plans based on patient needs and goals, coordinating with other healthcare professionals as needed.
  • Educate patients and families on disease management, treatments, medication adherence, and overall wellness.

• Case Management:

  • Monitor patient progress and adjust care plans as necessary, ensuring optimal patient outcomes.
  • Serve as a liaison between patients, families, and the care team, ensuring timely communication and support.

• Documentation & Charting:

  • Maintain accurate and timely documentation of patient care, assessments, and interactions in the EPIC system.
  • Ensure compliance with state, federal, and company guidelines and standards.

• Scheduling & Patient Visits:

  • Visit up to 6 patients per day, managing the travel and time efficiently.
  • Ensure all visits are completed within the scheduled hours and adhere to productivity standards (30 points/day).

• Travel & Transportation:

  • Must have reliable personal transportation to travel to patient homes, which may be up to 50 miles from the central location, with occasional longer distances.
  • Mileage reimbursement will be provided at the IRS standard rate.

• Experience:

  • Minimum 1 year of skilled home health experience is required.
  • Oasis experience is mandatory.

• Certifications:

  • Current BLS (Basic Life Support) certification is required.

• Skills:

  • Ability to work independently, manage time effectively, and maintain organization while caring for multiple patients.
  • Strong assessment skills, including the ability to complete Oasis assessments and other home health-related documentation.
  • Excellent communication skills to interact with patients, families, and interdisciplinary teams.

• Licensing:

  • Active and unrestricted RN license in the state of Washington is required.

• Travel:

  • Must have reliable personal transportation for daily travel between patient homes. A good portion of travel is within a 50-mile radius, though occasional travel beyond 50 miles may be required.

• Experience with EPIC for electronic charting is highly preferred.

• Shift:

  • Day shift (8:00 AM 4:30 PM), Monday through Friday.
  • Block schedules are not available for this position.

• Weekend & On-Call:

  • No weekend or on-call requirements.

• Mileage Reimbursement:

  • Mileage will be reimbursed at the IRS standard rate.

• Travel Radius:

  • Occasional travel of up to 50+ miles may be required, with the facility working to minimize drive time by grouping visits efficiently.

• Attire/Uniform:

  • Any basic scrub color.

• Charting:

  • EPIC for patient charting.

• Parking:

  • Free parking at the facility.

• Guaranteed Hours:

  • Guaranteed 40 hours per week, with the possibility of the facility calling off one shift every two weeks if needed.

• Pay Rate:
