- Conduct comprehensive assessments of patients and families to identify home care needs, including physical assessments and health history.
- Reevaluate patient nursing needs regularly.
- Develop, implement, and update individualized care plans based on nursing diagnoses, patient goals, and evolving conditions.
- Administer prescribed medications and treatments.
- Provide education to patients and families regarding care and health management.
- Identify and implement discharge plans as part of the care process.
- Serve as Case Manager when designated by the Clinical or Nursing Supervisor, coordinating care for assigned patients.
Communication:
- Maintain accurate clinical notes and update the primary physician as needed.
- Collaborate with physicians to report changes in patient conditions and obtain necessary orders.
Additional Duties:
- Arrange for equipment and services essential to patient care.
- Supervise, train, and evaluate home health aides.

- Active and unencumbered California RN license.
- Valid California driver's license and proof of auto insurance.
- At least one (1+) year of relevant nursing experience.
- Current CPR certification.
