Job Description:
The Registered Nurse (RN) with Homecare is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of Hospice/Home Health adult and pediatric patients and their families in their home setting. The RN provides the best clinical practice in an environment of caring and healing while working autonomously in the home. Utilizing the nursing process, the RN delivers care in compliance with hospice/home health policies and procedures, legal and regulatory standards. The RN utilizes the nursing process by use of technology, therapeutic intervention, evidence-based practice, and coordination of care with other health team members.

As a Homecare Registered Nurse, you will be responsible for providing comprehensive, patient-centered care to individuals in their homes, assisting with their medical needs, and helping manage chronic conditions. This role allows you to use your clinical knowledge to perform a variety of tasks including wound care, IV therapy, patient education, and more. The RN will be required to educate patients and caregivers on treatment plans, medication administration, and health management strategies.

- Current RN License in state of practice. License is verified
- Current BLS Certification for healthcare providers.
- Current drivers license, insured and reliable transportation and an acceptable driving record.
- Ability to communicate effectively both verbally and in writing
- Ability to transfer patients independently
- Ability to commute to the office at least twice daily to electronically transfer patient files or have other access to the Internet for electronically transferring patient files twice a day.
- RNs with less than 12 months of working experience as an RN prior to joining Intermountain must obtain their BSN within five years of their start date.
Preferred Qualifications
- Bachelor's Degree in Nursing (BSN). Education must be obtained from an accredited institution. Degree will be verified.
- Two years working as an RN.
- Previous experience in an Acute Care, Home Health, or Hospice setting.
Physical Requirements:
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, and driving a vehicle.
- Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
- Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of furniture, equipment, power cords on the floor, etc). Need to ascend and descend stairs or uneven surface in order to access patients.
- Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
