REGISTERED NURSE - Polaris Healthcare : Job Details

RN NEEDED FOR PER DIEM;

· NYS Registered Nurse

· This is a position that would go between our facilities in the region

· Must have recent experience performing and entering UAS assessments for adult patients (preferably in an assisted living setting)

o Assessments must be done in person during hours that are acceptable to the home/residents

o Paperwork can be potentially done remotely

· Prefer to have experience doing 4449c, MD order and interim order paperwork

· Experience in the Assisted living setting strongly preferred

· Experience in doing UAS assessment to achieve RUG scores as it relates to physical/occupational/ speech therapy STRONGLY preferred

· Experience with OASIS assessments preferred but not required
