Description
Callen-Lorde Community Health Center, a leading provider of LGBTQ+ healthcare, is seeking a dedicated Full-Time Registered Nurse (RN) to join our dynamic nursing team at our Brooklyn location. The RN will play a crucial role in supporting primary care providers by screening patient phone calls and walk-ins for urgent medical matters, triaging patients to urgent care, and referring patients to the ER or other facilities as necessary.
As an RN, your responsibilities will include:
+ Screen patient phone calls and walk-ins for urgent medical matters and triage patients appropriately.
+ Refer patients to urgent care, the ER, or other facilities as needed.
+ Provide patient education, adherence support, and case management assistance.
+ Follow up on laboratory calls regarding alert values and ensure timely patient communication.
+ Educate and support patients on a variety of medical issues, including HIV and HIV adherence.
+ Provide clinical support during patient care sessions when required.
Work Schedule/Salary:
Callen-Lorde is open Monday through Saturday. This is a full-time position that will require three morning and daytime shifts, two evening shifts, and rotating Saturdays.

Qualifications
+ New York State RN License and Registration
+ 1-2+ years experience in acute or ambulatory care.
+ Substantial experience in HIV primary care and sexually transmitted infection screening, treatment, and education
+ Experience in ER, telephone triage, public/community health care setting is preferred
+ RN Case Management and patient education experience preferred.
+ Current CPR and Infection Control certification
+ Ability to work with a diverse patient population and within a team-based structure
+ Bilingual (Spanish/English) preferred.
