Field Nurse, Home Care
Licensed Home Care Agency looking for full time RNs to provide superior healthcare services to patients in their homes. Responsibilities
· This role is crucial for ensuring our patients receive the highest quality care possible.
· The ideal candidate will possess a strong clinical background and demonstrate excellent patient assessment skills.
· The RN would be working under the direction of the DPS (Director of Patient Services) to evaluate clients in their homes.
· Conduct comprehensive assessments and aide supervisions in patients' homes.
· Perform thorough head-to-toe patient assessments, including medications and diagnoses, and document findings accurately.
· Supervise and support home health aides/personal care aides in all duties.
· Develop and implement personalized Plans of Care (POC) for aides to follow and provide orientation to the POC. These visits are to assess the client's homecare needs, create or adjust a Plan of care, and oversee that the caregiver is implementing the plan of care appropriately.
· Instructs and advise patients and/or families as well as the home health aides in their care.
· In addition, there is an occasional need to conduct visits in Orange/Westchester County when needed.
Qualifications
· Current and valid NYS RN License
· Current and Valid Driver's License
· Ability to travel to patient cases within Rockland County
· Must carry Medical Professional Liability coverage
· Experience with Medicaid, Medicare, and Managed Care.
· Experience in home and community-based or long-term care services.
· Proficiency in using the UAS-NY application.
· Experience working with vulnerable populations and a passion for patient care.
· Excellent written communication and documentation skills.
Work Location: In person
