Location: Richland, WA (Hybrid) Reports to: Director of Nursing (Savoy Life) Position Overview: Savoy Life is seeking a dedicated Registered Nurse (RN) to provide hands-on clinical support within senior living communities in Central Washington (Initial focus in Richland). This is a hybrid role where assessments will be conducted on-site at senior facilities, while administrative tasks and care coordination can be done remotely from home. The RN will work closely with on-site leadership to identify care gaps and implement chronic condition care plans to improve resident outcomes. Key Responsibilities:

  • Conduct comprehensive assessments of residents within senior living facilities, identifying care needs and gaps.
  • Assist in developing and implementing chronic condition care plans in collaboration with the clinical team.
  • Coordinate care across multiple stakeholders, including the facility's on-site teams and Savoy Life's medical group.
  • Collaborate with the Director of Nursing, Assistant Director of Nursing, and Health and Wellness Directors to ensure seamless care delivery.
  • Maintain accurate and detailed documentation in both the facility's Electronic Health Record (EHR) system and Savoy Life's Electronic Medical Record (EMR).
  • Provide clinical expertise and act as a resource for on-site staff to support high-quality resident care.
  • Operate with flexibility and adaptability in a new and evolving role within a growing organization.
Qualifications:
  • Active Registered Nurse (RN) license in Washington.
  • Must possess a valid and current driver's license.
  • Strong background in geriatric/senior care, with a focus on care assessments and care coordination.
  • Experience in care planning for chronic conditions, including collaboration with interdisciplinary teams.
  • Excellent documentation and electronic medical record-keeping skills.
  • Ability to work autonomously, solve problems, and adapt to new processes as the role and organization evolve.
  • Experience in working in senior living facilities is highly desirable.
Skills & Competencies:
  • Strong clinical assessment and care planning skills.
  • Ability to work closely with both clinical and facility teams to coordinate care.
  • Proficiency in using EHR and EMR systems for documentation and reporting.
  • Excellent communication skills and the ability to work collaboratively with a diverse team.
  • Adaptability in a new role and organization, with a focus on innovation and resident care.
