Implements and monitors patient care plans; communicates patient conditions as necessary. Coordinates care across all disciplines, and executes physician and nursing orders. Continuing education requirements as defined within the CARE program.* Preferred: Bachelors of Science in Nursing* *Required Licensure*: Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days* *Required Certifications*: Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) - American Heart Association, and others as required by individual department* Preferred: Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP) or others as required by individual department*Responsibilities:** Conduct initial assessment, re-assessment, and focused assessment based on patient condition, setting, and population.* Collect and analyze data about a patient including physiological, psychological, sociocultural, spiritual, economic, and life-style factors.* Identify problems by using clinical judgment about the patient's response to actual or potential health conditions or needs.* Set measurable and achievable short- and long-range goals in collaboration with the patient and family.* Formulate, implement, coordinate, and document patient plan of care.* Record assessment data, medication administration, ordered treatments, problem identification, nursing care, patient/family education, and patient goals in plan of care.* Conduct transistion or discharge planning accordingly.* Evaluate patient status and nursing care.
