A medical services organization in New York City is currently seeking a licensed Registered Nurse (RN) to join their growing team as their new Nurse Assessor. In this role, the Nurse Assessor (RN) will be responsible for performing initial in-home pre-evaluation of patients, including evaluation of health status, strengths, care needs, and preferences and guide the development of individualized long term care service plans. Responsibilities: The Nurse Assessors (RN) will: Review consumer medical documentation and or health referral forms, as relevant to the case Enter evaluation data into electronic evaluation form and transmit, as requiredDocument any concerns, conflicting information, other issues that surface during the evaluation processEmphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment planDiscuss with consumer on health care options, supports needed, service vendor options and waiver options Initiate communication between the Call Center and eligible consumers for enrollment into a managed care health plan Collect quality review data and any required documentation to support outcome measurements and record case notes into CRMIdentify opportunities for health promotion and illness prevention Perform other duties, as needed Qualifications: 2+ years of Nursing experience with Home Care background; or 1+ year of Nursing experience with UAS in Home Care Graduate of an accredited Nursing program NYS Registered Nurse (RN) license Desired Skills: Bachelor's Degree in Nursing Bilingual (English and Spanish, Russian, Haitian-Creole, Cantonese or Mandarin)
