Registered Nurse - PRN

Days or Nights/12 Hour Shifts/ Inpatient Center

Job Summary:

Primary functions are to administer nursing care for clients of all ages in the inpatient center, coordinate care with the interdisciplinary team, patient/family and referring Hospice of South Texas; and assume the responsibility for coordination of care.

Job Qualifications:

  • Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Licensure: Current Texas State license as a Registered Nurse, current Texas Driver's License.
  • Experience: Two years' experience as a Registered Nurse in a clinical care setting preferred, hospice preferred.
  • Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of Hepatitis consent/declination. Proof of current CPR. Specialized skills preferred in palliation and end-of-life care.
  • Transportation: Reliable transportation. Valid and current auto liability insurance.

Essential Function and Responsibilities:

  • Assess hospice patients to identify the physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations.
  • Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural/spiritual, verbal /non-verbal.
  • Consulting with and educating the patient/family regarding disease process, self-care techniques, end-of-life care, nutrition and dietary needs. Providing training to other staff as needed.
  • Implement/develop/document the plan of care to ensure quality and continuity of care.
  • Preparing clinical and progress notes that demonstrate progress toward established goals.
  • Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  • Provide clinical directions to the Hospice Aide/LVN to ensure quality and continuity of service provided.
  • Provide effective communication to patient/family, team members, and other health care professionals
  • Assure continuity of quality patient care delivered with appropriate documentation.
  • Monitor assigned cases to ensure compliance with requirements of third-party payor.
  • Demonstrate proficiency in all equipment used in patient care.
  • Demonstrate commitment, professional growth and competency.
  • Promote Hospice of South Texas philosophy and administrative policies.
