Runnells Rehab Center -

We are seeking a full-time, day shift Registered Nurse (Psych) to work in our Facility.

POSITION SUMMARY:

The Registered Nurse (Psych) assumes responsibility for a specific unit or area within the organization to include assessment (under the supervision of the DON), medication distribution, treatment clinical documentation, and supervision of IA's and other staff.

JOB RESPONSIBILITIES AND STANDARDS

Main Duties & Responsibilities:

· Complete nursing and mental health assessments

· Develop care plans and medical/psychiatric treatment plans for clients. Add items to the treatment plan problem lists

· Monitor po intake and escalate to dietician and physician when necessary

· Obtain vitals and recognize abnormalities

· Observe and report behavioral changes

· Provide nursing/psychiatric education to clients

· Conduct pain screens

· Conduct Communicable Disease Assessments

· Review and process new client intake medications

· Assist client that need to monitor BP, sugar, and other medical related items

· Conduct medication audits to track and monitor when refills are needed

· Liaison with pharmacy/collaborates with pharmacy for medication fills/deliveries

· Create lab orders based on physician requests

· Liaison to Lab for collaboration and scheduling

· Lab result review and triage for follow ups needed

· Wound care/basic life support triage and referral for Urgent Care/ER

· Referral and Care Coordination for Specialists (in conjunction with Case Management)

· Triage clients for Psychiatric APN assessments/appointments

· Track and report monthly PI data related to medical/psychiatric services (medications, infection control)

· Crisis de-escalation

· Work within a multi-disciplinary team to encompass a comprehensive approach for treatment of clients throughout the duration of their care episode

· Help clients improve their coping abilities

· Work with clients and their families to help improve the clients' abilities, and help them deal with family member's Substance Use and/or mental illness

· Record all care information accurately and completely within 24 hours in the appropriate format and on appropriate forms within the EHR

· Communicate eating disorder, medication non-compliance, suicidal/homicidal history and/or current concerns to the Clinical Director/designee accordingly

· 5. Demonstrate flexibility in assignment to meet resident/family needs.

· 6. Wear ID at all times appropriately when working.

· 7. Perform other position-related duties as assigned

QUALIFICATIONS:

  • Graduated from an accredited school of nursing and licensed by the state of NJ as a RN.
  • Current CPR or certified within 90 days of hire
  • Other certifications may be required for other areas of the organization
