Constant Care Health Services is currently seeking highly trained Registered Nurses (RN) for our 5 Star Skilled Nursing Facilities located in The Bronx. Our facilities are committed to providing high quality services and to do so we need amazing Registered Nurses (RN) .

Qualifications:

• Must have current New York RN license
• Must have BLS
• Must have experience as a Registered Nurse Preferred (New Graduates are Welcomed)
• Must have experience in Long Term Care Preferred
• Must have management experience
• Must have excellent communication skills

RESPONSIBILITIES:

• Performs required skills (assessment, intervention) according to guidelines, forms, policies and procedures
• Develops care plan that demonstrates knowledge of criteria to ascertain residents' specific needs based on health care problem(s)
• Carries out care plan that demonstrates familiarity with criteria to establish resident priorities specific to anticipated outcomes
• Implement physicians' orders; administer medications, start IVs, perform treatments, procedures and special tests and document treatment as required by company policy and local/state/federal rules and regulations
