This role involves developing care plans, coordinating discharge planning, and maintaining catheter care protocols.Responsibilities:Provide direct and individualized nursing care to patients based on the application of scientific nursing principlesAssess patient health problems and needs, develop and implement nursing care plans, and maintain medical recordsAdminister medications and treatments as prescribed by physicians or authorized medical personnelMonitor, record, and report symptoms or changes in patients' conditionsCollaborate with healthcare professionals to provide quality patient careEducate patients and their families on proper healthcare procedures and self-care techniquesAssist in the coordination of patient care, including discharge planning and referralsRequirements:Valid Registered Nurse (RN) license in New York StatePossess experience in a skilled nursing facility (SNF)Excellent knowledge of nursing care methods and proceduresDemonstrated experience with PICC line, IV insertion, blood draws, administration of IV fluids & antibiotics, and management of NG/PEG tubesProficient in various EMRs, such as SigmaCare, Epic, PointClickCare, Allscripts and WellSkyStrong communication and interpersonal skillsAbility to work well in a fast-paced environment and handle stressful situations with composureAttention to detail and excellent problem-solving skillsWork Shift:7:00 AM - 3:00 PM with every other weekend3:00 PM - 11:00 PM with every other weekend
