Position Summary:
The primary purpose of the RN Case Manager position is to provide ongoing support and expertise throughcomprehensive assessment, planning, implementation, coordination and overall evaluation/monitoring ofindividual patient needs. The overall goal of the position is to enhance the quality of patient management andsatisfaction, to promote continuity of care and cost effectiveness through the functions and integration of casemanagement, utilization review and management and discharge planning.
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Previous hospital nursing and/or Case Management experience preferred.
