Registered Nurse (RN) Director of Patient Services (DPS) for Rapidly Expanding NYS Licensed Home Care Agency
Job description
Home Care Agency is looking for an experienced Registered Nurse (RN) to oversee the clinical operations of a NYS Licensed Home Care Agency with multiple locations in downstate New York operating in all 5 boroughs of NYC, Long Island & Westchester.
As the Director of Patient Services, you will design, implement and manage the companys Paraprofessional and PDN (Private Duty Nursing) services, goals and objectives ensuring adherence to our policies and procedures.
Qualifications:
Responsibilities Include:
> Oversee and manage day-to-day clinical operations of the companys Home Care services (both Paraprofessional and PDN)
> Evaluates, supervises and monitors patient care services to ensure compliance with the Plans of Care/Treatment as well as companys Policies & Procedures
> Supervise companys Nursing Supervisors to ensure each is adhering to the companys Policies and Procedures, confirming all Caregivers (aides and nurses) are providing the care laid out in each Patient Plan of Care
> Works with clinical team to maintain and review schedules for all Field Nursing Staff, monitoring for timely completion of assessments, supervisions and ongoing MD orders
> Ensures availability of services and appropriate staffing to promote quality of care
> Perform ongoing assessments and manage patients health and welfarethrough coordination and scheduling of required services
> Conduct face-to-face meetings, schedule monthly team meetings to revieweffectiveness of Care Plans
> Participate in the development, implementation and evaluation of the companys Quality/Performance Improvement Program
> Performs annual reviews on all direct report staff (assessing staff competency)
> Conducts supervisory home care visits and, when necessary, hands-on care for those home care patients in need of said services
> Counsels and disciplines office staff, when necessary
> Responsible for Caregiver in-service training and new hire skill verification and development
> Office located in Manhattan location but must have the ability to travel and cover the companys coverage area of the 5 Boroughs of NYC, Long Island & Westchester
Compensation:
$120,00 - $150,000 per year Full-Time
