What does a Pinnacle Home Health RN do?

As a Home Health Nurse for Pinnacle Home Care, you will travel to a patient's home to administer services and provide a variety of tasks during patient visits depending on the specific plan of care and type of patient. Some tasks include:

  • Complete an initial assessment of patients and determine home care needs.
  • Develops a care plan involving patients and their family.
  • Medication/pain management
  • Re-evaluate patient nursing needs.
  • Initiate the plan of care and make necessary revisions.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

Qualifications:

  • Associate or bachelor's degree with One (1) to Two (2) years of recent acute care experience in an institutional setting.
  • Two (2) years of home health experience preferred.
  • Must be licensed in the state as a Registered Nurse (RN)
  • Current CPR certification
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills and nursing skills per competency checklist.
