*Job Title: Registered Nurse – Wound Care**Location*: New York, NY 10004*Pay Rate*: $90/Hr*Schedule*: 9:00 AM - 5:00 PM (Monday - Friday)_Note: 70% of the role is in-office; the nurse may be required to travel to patients' homes in Manhattan, Brooklyn, Bronx, and Queens to complete wound care consults. On average, the nurse will handle 15 to 20 consults a day._Job Summary:We are seeking a highly skilled and experienced *Registered Nurse (RN) with Wound Care Certification* to join our team. The RN will primarily manage wound care assessments and treatment plans for patients in the Home Care Program. This position requires a dynamic individual who can assess and create individualized care plans, collaborate with healthcare teams, educate patients and their families, and ensure optimal patient outcomes.Key Responsibilities:* *Wound Care Assessments & Treatment Plans*:* Conduct comprehensive assessments of deep wounds, skin tears, ostomies, pressure injuries, and diabetic foot wounds. Develop personalized treatment plans in collaboration with the healthcare team, considering the patient's physical, financial, and emotional resources.* *Collaboration with Interdisciplinary Teams*:Work closely with the home health team, including case managers, physicians, and nurses, to evaluate wound care patients and determine eligibility for the Home Care Program.* *Patient & Family Education*:Provide counseling and education to patients and their families about wound care, infection prevention, and treatment options. Ensure active involvement of patients and caregivers in the care process.Write clear and concise progress notes documenting the patient's response to care and treatment. Maintain accurate records in compliance with regulations and internal policies.Provide guidance, education, and validation of wound and ostomy care techniques to field nurses. Evaluate their skills during field visits and offer support in managing wound care cases.Participate in the development and revision of wound and ostomy management policies based on evidence-based practices. Engage in quality improvement initiatives and review clinical records for compliance.Ensure that all wound care practices align with state and federal regulations, as well as Home Care Policies and Procedures. Participate in audits and regulatory surveys as necessary.Required Qualifications:* *Documentation & Reporting*:* *Clinical Expertise & Mentorship*:* *Policy Development & Quality Improvement*:* *Regulatory Compliance*:* *Licensure & Certification*:* Current New York State Registered Nurse (RN) License.* Certification as a Wound Care Nurse or Wound Ostomy Care Nurse.* *Education*:* Bachelor's Degree in Nursing (BSN) required.* *Experience*:* facility.* At least 2 years of Public/Community Health experience and 2 years of medical/surgical nursing experience.* Experience with performance improvement (PI) and quality metrics.* Previous experience using electronic medical records and software applications (Epic, H2O, Microsoft Office Suite).* *Skills*:* Strong verbal, written, organizational, and analytical skills.* Proficient in using various computer systems and office equipment (Smartphone, laptop, desktop, copier).* Knowledge of PI tools, techniques, and the development of quality improvement programs.Additional Requirements:* *Travel*: Ability to travel within Manhattan, Brooklyn, Bronx, and Queens for home visits as required.* *Physical & Mental Requirements*: Ability to perform physical assessments and manage multiple cases simultaneously.
