Offering both traditional assisted living and assisted living with memory care, Linda Manor Assisted Living shares a campus with Linda Manor Extended Care, a skilled care center servicing the Northampton community since 1989.Essential Functions:Is knowledgeable of the individualized care plan for residents, and provides support to the resident in accordance with their care plan.Contributes to the care planning process by providing the nurse or other care planning staff with specific information and observations of resident's needs and preferences.Attends to the individual needs of residents, which may include assistance with dressing, grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, nail care, shaving, communicating or other needs in keeping with the individuals' care requirements.Maintain the comfort, privacy, and dignity of residents in the delivery of services to them.Maintain the confidentiality of all resident care information.Prepare residents for meals, serve food trays and assist with feeding as indicated.Records resident and resident-related events. Performs SAMM in accordance with facility policy and procedure.Reports any discrepancy noted with family filled medi-sets and pharmacy filled calendar cards.All prn narcotic medications are cued and signed off by two staff members. SAMM Discrepancy Reports are filled out for all Refusal of medication, Out of medication and Any reason a resident does not receive medications a s prescribed by physician.Documents all prn medications requested and effect of medication.Qualifications:High School Diploma or equivalentCertified Nursing Assistant certification preferred but not requiredCompletion and proof of a PCA/HHA training program that meets the 54 training hour requirement from EOEAPrevious experience working in long term care/assisted living community preferredPrevious experience working with dementia residentsSalary: $18.00 - $22.09/hr. depending on experience
