The Assisted Living Home is a 24/7 operated facility. Principle Duties and Responsibilities
Provide personal care services to residents for the purpose of maintaining health and independence. Assist with activities of daily living including but not limited to: bathing, feeding, dressing and grooming. Prepare meals, plan and coordinate menu with dietician, go grocery shopping and store items in cupboards. Perform general housekeeping duties including but not limited to: cleaning the kitchen, bathrooms and bedrooms including bed making, laundry, window washing, dusting, vacuuming, sweeping and mopping, Function as a team, assisting coworkers as the need arises. In the event that an assigned duty cannot be completed, ask for assistance and report to the Resident Care Coordinator. Communicate any concerns regarding resident changes in condition or mentation to the Resident Care Coordinator or Director of Nursing. Monitor resident activity, food intake, functional status, psychosocial status and reporting any changes immediately. Act immediately on any resident crisis, following protocol and basic first aid. Maintain communication with the Resident Care Coordinator and Director of Nursing regarding resident events, functions and outings. Follow the schedule of duties for the resident to include the Plan of Care and Safety guidelines to include: updating log sheets and resident documents, proper lifting technique, universal precautions and general safety. Attend meetings, care planning discussions or resident assignments regarding the Assisted Living Facility; participate in program planning, adhere to employee handbook and dress code, conduct and scheduling. Provide and receive in-service education and other training; monitor clients by performing blood pressure readings, glucose testing, medications, and general well-being of the resident. Document patient data into Electronic Health Record. Perform other duties of a similar nature or level as requested by supervisor or director. Required Knowledge, Skills, and AbilitiesKnowledge of:
- Basic health care practices and techniques;
- Community health activities and clinic routines;
- Problems, needs and behavioral health patterns of the disadvantages;
- Community health delivery system;
- Yaqui culture, customs, resources and traditions and/or a willingness to learn.

- Operate a variety of office equipment, including a computer and related software applications;
- Good communication and interpersonal skills as applied to interaction with co-workers, supervisor, management, Council members, and the public. Have ability to sufficiently exchange or convey information and receive verbal and written work instructions.
Education, Certifications and Experience Required High School Diploma or GED and Certified as a Caregiver, plus one (1) year work experience in a health-related field AND
- Bilingual in English/Spanish is preferred;
- Must possess and maintain a valid Arizona Driver's License.
Special Requirements:
- This position will require the incumbent to work non-traditional hours, nights, and weekends as this is a 24/7 operated facility. Shift differential available.
- Must have a current Level 1 Arizona Clearance Card or be able to obtain the Level 1 Arizona Clearance Card within ninety (90) days of hire. Failure to maintain a current Level 1 Clearance Card will result in removal from this position.
Shift Differentials:
- Afternoon Shift (2:00 PM - 10:00 PM):Receive an additional $0.50 per hour.
- Night Shift (10:00 PM - 6:00 AM):Receive an additional $0.80 per hour.
