Job Description :
HRM Services seeks a motivated, compassionate individual who will be responsible for case management, delivery of service-enriched programs, service record maintenance, preparation of program reports, and collaboration with various agencies, property managers, and others to achieve desired program outcomes. management and reporting, This is a full-time position with benefits.Essential Skills:

Responsibilities:
  • Conduct intake appointments upon resident move-in
  • Meet with residents on a regular basis.
  • Create and case manage resident service records.
  • Enter data and outcomes into an electronic reporting portal.
  • Build and maintain relationships with community service providers, property management and other team members.
  • Coordinate supports and promotes positive outcomes.
  • Assist residents in accessing community resources.
  • Facilitate resident meetings.
  • Encourage residents to be proactive toward addressing social, behavioral, and physical needs, maintaining self-sufficiency and housing stability.
  • Educate residents on service availability, application procedures, and residents rights and responsibilities,
  • Facilitate-e volunteer support, educational and recreational opportunities with community providers.
  • Other duties as assigned.
Qualifications:
  • A B.S. degree in a social services field is preferred, Applicants with of a minimum of five years' experience in human/social services or a combination of experience and education will also be considered.
  • An understanding of social service systems of care and accessing community resources is essential.
  • Reliable transportation and willingness to travel to both required sites.
  • Strong verbal and written communication skills needed.
  • Experience with file documentation and case management required.
