Resident Service CoordinatorCase Management without the travel! The Resident Service Coordinator is located in a senior housing complex. You will be providing information, referral, service coordination, activities planning, and follow-up to all building residents. Your caseload will include residents of the building, but also members of the Melrose community that are consumers of Mystic Valley Elder Services.Schedule: This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer 4-day work week (once training is complete.) No nights or weekends.Compensation: Starting annual salary is $53,845.25 Being bilingual is not a requirement for this role, but we recognize it as an asset and offer an additional 6% bilingual designation for Spanish, Haitian Creole, Russian, Vietnamese, Cantonese or Mandarin. Additional duties may be assigned as required.Conduct initial and on-going assessment of consumers' health and functional status.Conduct visits as required in various settings including consumers' homes, community settings, and/or medical facilities.Participate in development of consumers' care plans, including determining appropriateness and need for in-home/community-based services.Arrange, coordinate, and oversee in-home and community-based services for consumers.Maintain consumer records in accordance with standards.Compile and submit required reports.Coordinate activities and presentations for residents.Collaborate with management of the housing company/owner.Collaborate with contracted provider agencies to coordinate and assign service staff to consumers/residents.Qualifications:A Bachelor's degree in Social Work or a related field.Social Work license preferred.Knowledge of and experience working with older adults in a community-based setting preferred.Strong interviewing and assessment skills.Strong interpersonal and organizational skills.Excellent written and verbal communication skills.Computer skills: Proficient with MS Office Suite and entering narrative and other data into a database. Ability to use multiple databases.
