POSITION SUMMARY
The Resident Services Coordinator (RSC) works directly with the residents of the Independent Living Apartments (ILA) to enable them to live safely and independently (with or without assistance) in their own homes for as long as possible. The RSC provides general social work services to residents of the ILA including outreach, information and referral to appropriate community resources, comprehensive assessments as appropriate, care planning, short-term counseling to residents and family members, and case management to frail and at-risk residents as needed. The RSC conducts community needs assessments and acts as an advocate for residents' needs within The Redwoods and the larger community. This is a 3 day (24-hours per week), Saturday through Monday, position.
PRIMARY DUTIES AND RESPONSIBILITIES
- Case management: conduct assessments, develop individual service plans, and coordinate services.
- Maintain accurate, current, records and files in accordance with California state law.
- Facilitate and support neighbor-to-neighbor and family care-giving.
- Participate in the orientation of new residents to ensure that new residents have necessary information and support to make a successful transition to The Redwoods.
- Liaise and advocate within The Redwoods and with outside social services and health agencies.
- Proactively identify resident needs, provide and coordinate services and monitor their status.
- Assist residents and their families in identifying needs and preferences.
- Prepare the new move in folders and new move in packets for department intake, and other department flow required paperwork.
- Other duties as assigned.
QUALIFICATIONS / MINIMUM REQUIREMENTS
- Three years of experience in a social service capacity.
- Excellent writing, documentation, and organization skills.
- Strong case management, assessment and care planning skills.
- Knowledge and understanding of the problems and realities involved in management of retirement communities.
- Sensitivity and understanding of the issues related to aging.
- Strong working knowledge of social, medical, and legal issues impacting the elderly.
- Knowledge of relevant community resources.
- Instructional skills to present information to individuals, families, staff and groups in a clear manner.
- Ability to organize and prioritize workload, document services provided and develop care plans.
- Ability to relate appropriately and effectively to residents and their families, to other staff as part of a multidisciplinary team, and to personnel of operational departments.
- Must demonstrate excellent skills in reading writing, speaking and understanding the English language. Other languages a plus.
EDUCATION
- Master's degree in social work, gerontology, psychology or related field required.
PHYSICAL REQUIREMENTS:
- The position requires ability to retrieve work materials, the use of basic office equipment; intermittent walking, and standing, stooping, carrying and lifting of materials and supplies up to 35 lbs.
- Computer typing is a key part of the position.
- Work is primarily performed in an office environment with minimal exposure to health and safety considerations however, contact with frail residents and the general public is a key part of the position. Infection control practices must be followed as prescribed.
- Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
