Job Description

POSITION TITLE: Residential Coordinator

DIVISION: Community Living

SUPERVISOR: Regional Assistant Director, Community Living

CLASSIFICATION: Exempt

STATUS: Full-Time

PURPOSE:

Provide supervision to the Team Managers to achieve the goal of ensuring that the client receives necessary services and interventions. Ensure caregivers are trained and capable of delivering the needed services to clients. Guide, train and oversee staff are using online scheduling tool.

ESSENTIAL FUNCTIONS:

* Supervise Team Managers. Evaluate the performance of Team Managers at 90 days and annually. Assist Team Managers with reviews of DSPs at 90 days and annually, as needed.

* Audit and approve the time of Team Managers. Assist Team Managers in auditing and approving DSP time, as needed. Audit and approve mileage for all Supported Living staff.

* Meet weekly with the Team Managers to discuss clients and staff related issues as needed. Conduct monthly Supported Living staff meetings for training purposes, and to update the staff on department changes and upcoming agency events

* Ensure services are provided in compliance with all funding sources and governing bodies (CARF, BDDS, BQIS, State and Federal Law).

* Meet monthly with the Regional Director and Senior Director, Residential Services to discuss personnel related issues, client programming and compliance/incident reporting.

* Assist the Director of Community Services, as needed.

* Serve as a liaison between clients, families, staff, external professionals, and the community.

* Assist with the review of billing and time entry for accuracy and compliance with regulations.

* Provide direct care as needed and participate in the on-call rotation.

* Display patience in all situations involving clients. Speak in an even, positive, and professional tone of voice when conversing with clients.

* Appropriately respond to client's behavior via verbal and/or physical intervention based on the needs of the client.

* Be readily available to answer staff questions and/or schedule time to meet with Residential staff and route them to the appropriate department, if needed.

* Plan, schedule, and conduct divisional orientation with new Residential Staff.

* Provide training for agency staff covering topics required by regulation and quality service provision. Apply a variety of training methods as appropriate, including lecture, role model, text, and outside sources.

* Strictly maintain confidentiality of all clients and staff.

* Ensure that employees follow all departmental and agency policies, as well as state and federal laws. Conduct review of policies.

* Review the hours planned and scheduled as needed to ensure client budgets are being utilized in accordance with their allocations with the online software, Accel and Provide.

* Monitor and document all staff PTO, Medical time and leaves of absence.

* Maintain working knowledge of Provide system. Lead and attend trainings as needed with the IT department.

* Assist with special projects, as needed.

* Assist in setting up new client home files as requested and ensure that all files are maintained as needed. Ensure that all files are kept in order and that documentation is completed, filed and copies sent to be uploaded in Provide. Complete file audits in accordance with departmental, state, and federal regulations monthly.

* Assist with home visits and ensure that staff members are maintaining records that document client health care, active treatment, social information, and protection of client human and civil rights.

* Be willing to be flexible in work hours, including some evening and weekend hours for the purposes of reviewing program area and observing staff on duty.

* Assist in planning and providing instruction in communication skills, daily living skills, socialization skills, fine and gross motor skills, perceptual skills, prevocational skills, and any other areas based on programming needs.

* Follow all agency and departmental policies and procedures.

* Prioritize multiple job functions, coordinate routine responsibilities with spontaneous, work assignments.

OTHER FUNCTIONS:

* Promote agency philosophies, generate team spirit, create and maintain an environment that fosters employee satisfaction, maximizes productivity and profitability.

* Assist with planning and participate in all special Community Living activities, as needed.

* Must be able to care for the physical needs of clients including feeding, hygiene, and toileting. Must be capable of assisting in physically transferring/moving clients when needed. Physical activities may include but not be limited to: lifting up 25-50 pounds (depending on client); bending; reaching; stretching; grasping; visual acuity; walking; squatting, guiding; and transporting. Must be able to handle physical aggression as defined in NCI training.

* Assist with the use of medical adaptive equipment including, but not limited to: Wheelchair, Hoyer Lift, Shower Chair, Blood Sugar Testing Equipment.

* Coordinate and transport clients to medical appointments and/or procedures as needed.

QUALIFICATIONS:

* Bachelor's degree or equivalent experience in a human service field preferred, plus at least one year of experience working with the DD population, preferable in a residential setting.

* Must possess excellent communication skills both written and oral, and strong teamwork skills.

* Must have the ability to successfully work in both independent and group settings. Must be capable of communicating effectively during a crisis situation.

* Must be able to work a flexible schedule, including some evenings and/or weekends, to meet the training needs of divisional staff.

* Maintain a valid Indiana driver's license, insurance on personal vehicle (minimum of PLPD with Medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier.

* Must possess and maintain reliable transportation that meets state requirements for transporting clients.

* Possess and maintain a clear criminal record as required by Medicaid Waiver regulations.

* Must possess or be able to obtain CPR and first aid certification annually and perform as necessary.

* Must possess or be able to obtain NCI certification annually and perform as necessary.

* Dress appropriately as defined by department dress code.

* Must be able to perform such functions as operate a computer, access existing records storage, attend meetings throughout the community, and lift up to 25 pounds.
