Resource CoordinatorPay: $22.00/hrThe Resource Coordinator is responsible for reviewing independent assessments to determine appropriate resource referrals for clients. This role will report directly to the Program Director or Direct Manager (if applicable) on all matters pertaining to their duties. The Resource Coordinator works collaboratively with clinical teams to ensure that multiple client needs are addressed through individualized care plans and appropriate referrals. This position requires a strong working knowledge of clinical modalities, such as motivational interviewing, cognitive-behavioral strategies, and medication-assisted therapy.Training
  • Attend and complete all required trainings as assigned by the manager.
Duties and Responsibilities
  • Review independent assessments to determine appropriate resource referrals.
  • Develop clinical hypotheses targeting clients' specific service needs.
  • Assist in the development of individualized Plans of Care.
  • Track clients in the Opioid STR program and log/report data as required.
  • Participate in recruitment and referrals for the Opioid STR program.
  • Oversee care coordination processes among participating agencies.
  • Ensure multiple client needs are addressed, and appropriate referrals are made.
  • Promote identification, engagement, and retention of clients.
  • Maintain current knowledge of available services in the surrounding area.
  • Coordinate resources through various entry and delivery systems for clients' wellbeing.
  • Maintain knowledge of clinical modalities such as Motivational Interviewing, Cognitive Behavioral strategies, and Medication-Assisted Therapy.
Minimum Requirements
  • Education: Bachelor's degree or higher in a relevant field.
  • Experience: Minimum of two years of professional experience, particularly in project management or community/public relations (behavioral health experience preferred).
  • Skills:
    • Knowledge of evidence-based treatment options and systems.
    • Familiarity with opioid addiction, substance abuse treatment, and related stakeholders (e.g., child welfare, juvenile justice, behavioral health).
    • Proficiency with Microsoft Office Suite and Outlook.
    • Strong organizational, communication, and leadership skills.
    • Ability to work independently and as part of a team.
    • Ability to travel across the region as needed.
