RN Admission Care Liaison - Real Time (Tele-Health) Care FT - Ohio's Hospice : Job Details

What You Should Know About the Real Time Care Role:

• This is a FT M-F position, 4-10hr shifts per week, 11am-9:30pm with 1-2 holidays per year
• We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed

The Real Time Care Essential Duties Are:Ohio's Hospice patients will receive quality nursing care and coordination of hospice services for the patient and family with the appropriate resources needed at the end of life. Qualification:

• Active Registered Nurse license in the State of Ohio without any board actions.
• Two years acute care nursing experience preferred
• Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
• CPR Certified
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage
