Northtowns Care Center, a skilled nursing facility in Buffalo, New York is currently looking for an RN Admissions Nurse to join our team of outstanding professionals. Our goal is to create an employee supportive environment utilizing staff-friendly strategies and a team-oriented approach to make sure our staff are well-trained and our residents are well-looked after and receive top notch care. The RN Admission Nurse is responsible for admitting all new patients including nursing assessments to the facility while ensuring the nursing operational standards of the facility are met Monday through Friday from 3 pm until 10 pm .

RESPONSIBILITIES:

  • Coordinates prospective admissions with the Director of Nursing, and Administration.
  • Obtains and completes all necessary admission and readmission paperwork.

Ensures that all follow-up consults are created for the Unit Secretary to schedule in a timely fashion.

Completes all nursing assessments including initiating the Molst.

Ensures Facility compliance with all state requirements and facility policies, procedures and standards of conduct.

  • Have the ability to assess, plan, implement and evaluate nursing care according to the individualized needs.
  • Communicates and collaborates with the interdisciplinary team and or physicians.
  • Completes all paperwork for the Nursing Admission Assessment.
  • Enters all data into Electronic Medical Record.
  • Obtains orders from MD and ensures accuracy of Admission Orders in EMR.
  • Completes the Admission Nursing Note.
  • Completes the Fall Risk Assessment and interventions care plans as indicated (sensors, alarms, etc).
  • Completes the Pressure Ulcer Risk Assessment / Braden Scale. If high risk ensures all preventative measures are implemented.
  • Obtain consents: Pneumovac / Flu and places in patients chart.
  • Ensures Initial/Interim Care plans are put in place. Fall Risk, Skin Alt-Potential, Pain specific resident conditions (pressure ulcer, post op, infection, etc.,).
  • Administers 1st dose of PPD within 24 hours and records on Immunization Record and Med sheet.
  • Ensures all paperwork for the admission is complete and signed off on.
  • Must have strong communication skills.

QUALIFICATIONS:

  • Current RN license
  • Long term care experience preferred, with admissions experience
  • Reliable transportation and demonstrate excellent work ethic.
  • Must have excellent interpersonal, communication and organizational skills.
